Defects are set up by the Contract Manager.   To get started, click the **Add** icon at the top of the **Defects** table. ![](https://support.surepact.com/hubfs/Knowledge%20Base%20Import/surepact.zendesk.comhcarticle_attachments8896733498127-2.png)   Assign a name and description for the defect, along with the other required fields. The **Delivery Owner** is the team member responsible for completing the delivery Click **Save Changes**. ![](https://support.surepact.com/hubfs/Knowledge%20Base%20Import/surepact.zendesk.comhcarticle_attachments8896733498383-2.png) *Note: If the Delivery Owner is different than the Delivery Creator, the Delivery Owner will be [notified](https://support.surepact.com/surepact-notifications?hsLang=en) of their new assigned task. Both the Delivery Owner and Delivery Creator can enter or edit fields for the defect.*   Once saved, the Delivery Owner can continue with the defect tabs: **Details**, **Documents**, **Approvals**, **Submit**, **Reports**, **Permissions,** and **Governance Log**.
